At PhysioMax Wellness, we strive to make accessing your healthcare as seamless as possible. We offer direct billing to various insurance providers to save you time and reduce out-of-pocket expenses.
Direct Billing and Insurance Guidelines
To ensure a smooth experience, please read the following:
• Direct Billing Authorization: By signing our authorization form, you permit PhysioMax Wellness to submit claims directly to your insurance company. This includes the release of necessary information about your policy.
• Coverage Responsibility: It is your responsibility as the policyholder to track your coverage limits and understand your benefits.
• Disputes and Audits: Any disputes or audits with your insurance company are solely the policyholder’s responsibility.
• Balance Payments: You are responsible for covering any balance not covered by your insurance plan. We may not be able to bill under secondary insurance.
• Missed Appointments: Missed appointments (without at least 24 hours notice) are not billable to insurance and will result in cancellation fees billed directly to you.
• Coordination of Benefits: PhysioMax Wellness does not handle coordination of benefits between multiple insurance providers.

How to Get Started
• Complete our Direct Billing Authorization Form at your first appointment.
• Provide all necessary insurance details, including policy numbers and member IDs.
• We’ll verify your coverage before your visit and inform you of any potential costs such as copays, deductibles, or coinsurance.
